CYS Membership
Registration Night for 2008-2009 will be held on Thursday, September 4th, 7:00pm - 9:00pm at Wesley United Methodist Church.

Returning families should complete and come with the following forms (see below):
  • Registration Form (3 copies per participating child)
  • Emergency and Consent Form (One copy per child per activity, plus one additional copy for Membership)
  • Waiver and Ethics Forms (One copy per family)
New CYS Families should also complete the New Member Application.

Registration Forms
On this page, you will find the various forms required for membership in CYS. All of the forms require Adobe Reader. Those forms that require data entry are "interactive"; you can type in your information, then print it off.

CYS Handbook
The CYS Handbook describes the CYS organization and its rules and policies.

New Member Application
The New Member Application is required for new families that are entering the organization. One form is required per family, but only when you first join the organization.

Registration Form
The Registration Form is required each year to insure that we have your correct information, and to sign up your children for the various CYS activities. Three copies of this form are required per child. You will turn in three of these copies (for basketball, membership, and treasurer) and one copy is for you to record your manpower signup. This form changes each year, so please make sure that you are filling out a current version of this form.

Emergency and Consent Form
The Emergency and Consent Form has the emergency contact information for your child, and is another form that is required each year. One copy of this form is required per activity for each child and one additional copy is required for Membership. Please use an original signature on each copy of this form.

Waiver and Ethics Form
The Waiver and Ethics Form is also required each year. Only one copy is required per family. Please make sure that the participating children read, understand, and sign the ethics form as well as the parents.
