Below you will find the various registration forms required for membership in CYS: ¥ Emergency and Consent Form All forms are interactive PDF's. While they can be viewed and printed normally, they can also be filled in electronically and then printed. Interactive use requires Adobe Reader.

Emergency and Consent Form The Emergency and Consent Form has the emergency contact information for your child. This form and all of it's copies must be submited each year. One copy of this form is required per activity per child PLUS one additional copy per child is required for Membership. Original signature on each copy. The rest of the forms are down and under construction in preparation for the 2010-2011 registration cycle. Check back in July 2010.
