2011 - 2012 Registration 2011-2012 registration will once again use a web based registration solution. Please read the information below for important dates and what to expect. The most important item is your primary email address. Your primary email address is used to log into the new system and is our main avenue to provide you with instructions and information. Registration Calendar (2011-2012 Key Dates) August 31st 12:00 PDT Online Registration for all members. !! NOW OPEN !! --->Online Registration Link <--- FAQs (Frequently Asked Questions): Q. I don't have a password yet, Do I need one? A. The website incorrectly noted that UserIDs and passwords would be emailed out in advance. Passwords will be reset/set upon initial access to the registration website. Q. Will we be able to forward a UserID to new prospective member? A. No, this is not the process.? All New Members must follow the new member process. Q. What is the new member process? A. New members must submit a New Member Application. Upon acceptance and processing an online registration UserID will be emailed to the primary email address for your family. If you can not attend this new member orientation you will need to contact Membership and make arrangements for submitting the following: Q. Do New Members need to submit a Registration Form? A. No this is for returning members only who will be making alternative payment arrangements and thus NOT registering online. This requires prior approval. Q. Where / When is New Member Orientation Night? A. New Member Orientation Night was held August 23rd. Contact Membership if you're a new member and missed event. Q. How will I be able to sign up for Manpower? A. Manpower lines will be superseded by the automated registration process. Manpower will be based by the age of your oldest child as per the normal process. The registration website will direct you to your available assignments. Audits will be completed by Manpower and confirmation or changes to your manpower assignment will be sent via email after registration. Q. How can I pay for registration? A. Credit card payments are preferred as listed on the registration form. Other payment options will be available. FORMS CYS Handbook The CYS Handbook describes the CYS organization and its rules and policies. (revised 8/19/11) Emergency and Consent Form The Emergency and Consent Form has the emergency contact information for your child. A copy MUST be submitted to the coach/commissioner on the first day of each activity. One copy of this form is required per activity per child. . Original signature on each copy. New Member Application The New Member Application is required for new families that are entering the organization for the first time. It is not required for returning families. One copy per family. All forms are interactive PDF's. While they can be viewed and printed normally, they can also be filled in electronically and then printed. Interactive use requires Adobe Reader.
